2009 CX Debate State Meet - Information Form for Judges Hired by UIL

You should only fill out this form if the UIL State Office has already confirmed that you will be hired by the League to serve as a UIL Hired CX Judge. If you are interested in judging but have not received an invitation or been previously contacted by the UIL office, you first will need to complete the Judge Interest Form and then follow-up with an email to UIL Speech & Debate notifying us of your interest in judging at the CX Debate State Meet.

IMPORTANT INSTRUCTIONS (Please read entirely before beginning!):
Thank you for agreeing to judge at the 2009 UIL CX Debate State Meet. Please complete this form by Friday, February 13, 2009.

We are required to have a Payee Form (PIF) for all judges being paid by UIL. In addition, we must have a photocopy of your driver's license or Social Security card.

The PIF requires a signature so you will need to return it via fax: (512-232-7311) or by mail:
