Please complete this form if you have not done so at the conclusion of each football season.

Be prepared to fill in ALL requested information. You must complete and submit all information at the same time in order for your form to be processed properly. Please type using normal case; do not type in all caps or all lowercase.

Use your Tab key to move to the next blank. DO NOT hit your Return or Enter key, as this will cause your form to be submitted prematurely.

After you have completed the form, you MUST click the Submit button in order for your information to be sent to the UIL office. Be sure to review your information carefully before clicking Submit.

List games in chronological order. Type dates using ONLY numerical "month/day" format (example - 10/26). Dates MUST be in this format using numbers only; entering text in the date column will result in an error message and your form not being processed. It is not necessary to add on "High School", "HS", or any other notation when you enter the opponent schools. Please enter both city and school even if the names are the same. List game scores with your score in the first column as indicated. Use the last column to specify an overtime game or a forfeit where applicable.
