IMPORTANT: As soon as the district championship is determined, please certify to the League office by letter, by fax on the printed form provided in the district chair manual, or on this form. PLEASE DO NOT DUPLICATE REPORTING METHODS - IF YOU CERTIFY ON THIS FORM, YOU DO NOT NEED TO FAX, CALL OR WRITE THE UIL OFFICE.

If you are certifying a Conference 1A-Eleven Man or 2A, 3A, 4A or 5A district, please use the 1A-Eleven Man-5A certification form.
